You will soon be receiving an invitation from our new nurse, Sarah Wood, who has been employed especially to look after you, if you are over 75. Sarah has expertise and knowledge about services and help you may wish to access, that you do not presently know about. When you meet Sarah she will do a general assessment and advise you on what is available to you should you need extra help. If you are fit and active this is a great opportunity for you to have an over 75 well person check. Most of you will be fit enough to attend the surgery, but for those of you who are not and are housebound, Sarah will visit you at home.
For our over 75 patients that are not quite so well and need a little extra help, a Community Matron has been employed, she is called Diane Hobro and she will be looking after patients from Pendleside, Castle, Whalley and Slaidburn practices with complex needs such as multiple long term conditions.
All our patients are registered with the practice but have a doctor that they usually see when they come to the practice. It is especially important to us that our patients who are over 75 know which doctor to contact should they need them. You have all been informed of who your usual doctor is, but if you are still unsure please contact us at the practice or ask Sarah or Diane when you see them.
The practice wants to ensure that our elderly population is well cared for and that you are getting all the help you need.
