Providing you live within our Practice area, you can register with us by completing a Registration Form and New Patient Questionnaire. Once filled in, bring them in and hand to the reception desk. You will be registered within two working days, then your records will be transferred to us by the Health Authority from your previous practice.
Please note you WILL NOT be registered at the Practice unless you complete a New Patient Questionnaire and Registration Form.
If you are on any repeat medications, please ensure you are booked in with one of our nurses to review your medications.
Your records from your previous practice may not come to us straight away.
