If you decide to pursue an ADHD assessment through the NHS, please be aware that waiting lists can often be quite long.
Right to Choose (RTC) Pathway
Alternatively, you may wish to use the Right to Choose (RTC) pathway. This option allows you to select an approved private provider for your assessment, with the cost covered by the NHS.
Important: You must choose your own RTC provider — your GP cannot do this on your behalf.
Choosing a Provider
Before Selecting a Provider
Before choosing a provider through the Right to Choose pathway, please make sure that:

✅ You meet their referral criteria

✅ They are able to prescribe medication if needed

✅ You are aware of their current waiting times
⚠️ Tip: Don’t choose a provider based only on the shortest waiting time — make sure it’s the right fit for your needs.
Important: NHS GPs are not obligated to carry out additional tests or prescribe medication following a private consultation.
How to Proceed with a Referral
To request a referral, please complete the below online referral form:
Alternatively you can print and complete the following documents and send them to us:

ADHD screening questionnaire (completed) - We can provide this questionnaire either as a digital link sent via text message or as a printed paper copy. If you prefer to receive it by text, please inform the receptionist when booking your appointment. Alternatively, you can request a paper copy to complete and return to us.

Personal statement detailing:

Your symptoms
-
How they affect daily life (home, work, relationships)

What you've tried so far
-
What you hope to gain from a diagnosis
(Please be as detailed as possible – inadequate information may result in your referral being rejected.)

Name of your chosen Right to Choose provider
- Completed and signed patient disclaimer form
We can only begin your referral once all documents have been received.
Deadlines and Reminders
If any documents are missing, we will send reminders and hold your request for 28 days. If we do not hear from you within that time, we will assume you no longer wish to proceed, and your referral request will be closed.
After Submission
Once all documents are received:

A GP will review your submission

If you meet the referral criteria, our reception team will be in touch to arrange an appointment with a clinician
